Youth Support Services - Case Manager
Permanent Part Time

The Case Manager will work within the Youth Support Services program and ensure that all identified targets are met in line with the specified service agreement and outcomes of the program. The Case Manager is responsible for the day to day delivery of services to the specified target group (young people aged 8 - 21yrs of age) and ensuring that these young people receive targeted, person centered intervention, using a flexible and holistic service delivery model. The position is required to provide intervention for Youth in the specified geographical catchment and deliver outcomes in the following four areas:
- experience positive relationships with their family and community
- are engaged in education, training and/or employment
- lead healthy and violence free lives
- have safe and stable places to live.
The role's aim is to work towards improving the lives of vulnerable youth in the area and engage young people who are most at risk in the local community.
Key Responsibilities Include:
- Align ones workplace behaviour in accordance with the principles, values and Mission of the organisation.
- Work from a strengths-based perspective to assist young people and families with complex issues to meet their identified needs.
- Assess and case-manage young people creating appropriate case plans and interventions as needed.
- Assist young people to identify, develop and improve living skills, and assist them to link with local support networks.
- Support and facilitate access to appropriate support groups and other referral sources in relation to their identified needs.
- Develop and maintain partnerships with local internal and external stakeholders.
- Maintain comprehensive and up-to-date client documentation as required under the terms of the funding contract, and within organisational policy.
- Provide regular reporting to the Management on relevant areas of program functioning and monitor performance indicators.
- Participate in program development, evaluation and other quality related initiatives including accreditation activities and data collection.
- Follow risk management policy and processes identified for the program.
