Recovery Support Worker

The Recovery Support Worker is responsible for delivering individualised recovery supports for 14 tenants living in the Clear Breeze Apartments.
The Recovery Support Worker is required to work alongside the Mental Health Recovery Workers and Senior Worker to deliver flexible and responsive daily living supports which promotes individual choice, self determination, independence and least restrictive practice in accordance with client goals. Clear Breeze Apartments are a purpose-built supported accommodation model for 14 adults living with mental illness
Key Responsibilities Include:
- Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values.
- Ensure individual and team practice is in accordance with the requirements of relevant legislation including the Human Services Quality Framework and the National Mental Health Standards.
- Work in accordance with psychosocial rehabilitation, social inclusion and recovery orientated practice frameworks.
- Provide individualised goal oriented support as directed by tenants' recovery and NDIS support plans.
- Work as part of a 24/7 roster which includes overnight sleepover shifts in a dedicated staff unit.
- As required perform care and support coordination to support clients to arrange and manage outings, meetings, medical appointments, personal care tasks and other daily living tasks including housekeeping, shopping and cooking based on their personally identified needs and recovery goals.
- Ensure client information is up to date and in accordance with case requirements. As necessary draft documentation for a range of stakeholders.
- Undertake professional development which includes regular supervision and reflective practice.
- Participate in client case reviews within and external to Wesley Mission QLD to ensure that client service continuity, an individualised approach to services and clear pathways of transition to a more independent life is achieved.
- Identify and assist in locating resources, services and supports that facilitate community integration including social, educational, and vocational and leisure activities.
- Where appropriate, participate in the development and implementation of quality and service improvement activities that continually improve the service we provide.
