Family Support Worker - Youth At Risk Alliance

Family Support Worker, Youth At Risk Alliance
Permanent Part-time - 72 hours per fortnight
Social, Community, Home Care & Disability Services Industry Award - Level 5
Key Responsibilities Include:
1. Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values.
2. Supervise, support and assist the target group to achieve their case management goals.
3. Provide intensive in-home support, mentoring and role-modelling to families where there may be parenting difficulties and that are case managed by the Panels.
4. Cooperate, liaise and negotiate with key stakeholders in conjunction with, and on behalf of, the client.
5. Where appropriate, transport clients to and from appropriate activities and appointments within, and outside of, business hours.
6. In conjunction with the Panel Coordinators, facilitate and support group-based or experiential programs.
7. Keep appropriate, accurate and professional case management notes that can be submitted as admissible documents.
8. Implement assessment procedures and protocols in line with Program requirements.
9. Provide reports on clients to other team members, Panel Coordinators and Panels when required.
10. As appropriate ensure the safety of the client, and others
