Youngcare Wesley Mission Queensland Apartments Coomera on the Gold Coast offers a supported, vibrant community and home to people living with a disability who have high and complex care needs.   We are currently seeking a Community Access Facilitator (Full Time) to develop and implement community access experiences that are the choice of our residents.

This role is pivotal to the continued growth and development of the Community Access programs for our residents. You will ensure the team provides a positive and person-centred service to enable individuals with disabilities to reach their own potential and become integrated into their community. 

 

Key Responsibilities Include:

  • Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values.
  • Oversee the development of NDIS Community Access programs and plans for each resident.
  • Liaise with residents/primary support networks to develop and implement and review NDIS Community Access plans monthly.
  • Develop, submit and oversee NDIS Community Access Service Agreements.
  • Document and report ongoing progress and budget of resident's Community Access programs.
  • Complete risk assessments for all new community access activities for individual/groups of residents and review existing risk assessments annually.
  • Engage and train personal carers/volunteers to provide a high-quality community access experience either in house or in the community.
  • Access and support volunteers and link them to the needs and interests of residents.
  • Oversee and mentor personal carers/volunteers involved in the community access program.
  • Liaise and engage with Support Coordinators in the development of the Community Access Program for each resident.
  • Liaise and engage external organisations/providers to meet the individual needs of residents, so they can attend mainstream community access activities, with appropriate supports either internally or externally.
  • Ensure the team, activities and programs are in accordance with relevant WMQ policies and procedures.   
  • Attend and participate in regular meetings held by the Community Access Coordinator and other Community Access Facilitators to enhance program delivery.
  • In consultation with the local Manager order and maintain necessary equipment in line with operational and budgetary constraints.
  • Ensure adequate staffing levels are always in place by coordinating staff rosters in compliance with the relevant Award and client individual funding packages.

Essential Requirements Essential: Must have a current Drivers licence and willingness to drive in the course of work.
Essential: Qualifications in Leisure and Lifestyle or other relevant specialist area
Essential: Demonstrated experience working with people with a disability.
Essential: Demonstrated experience in the planning, implementation, facilitation and ongoing review of innovative and flexible recreational programs.
Essential: Ability to lead a small group of support workers and volunteers to deliver community access programs and activities.
Essential: Current First Aid Certificate
Essential: Relevant probity checks required by legislation and WMQ policy
