Client Liaison Officer Allied Health Community (Fulton Wellbeing Centre)

Role Purpose and Position Summary:
• To assess and process referrals for allied health and day respite services, including determination of service eligibility. • Ensure that prospective clients are placed in services which meet their needs as quickly and efficiently as possible. • Consult with the CLO Team Leader to assist with prioritisation of referrals. • To promote WMQ's services by networking with other Client Liaison Officers across Wesley Mission Queensland's other aged care services (including residential aged care and Community Care).
Key Responsibilities Include:
- Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values.
- Determine client eligibility for allied health services. If client is ineligible, it is the responsibility of the Client Liaison Officer to provide further advice regarding appropriate alternative services to potential clients/carers and refer on where appropriate.
- In collaboration with the CLO team leader, maintain a referral prioritisation list based on client clinical need.
- Efficient appointment scheduling and resource allocation for clinical and assistant staff.
- Development and maintenance of accurate electronic records.
- Conduct intake processing and initial assessments for new clients.
- Administration duties associated with client service delivery, including fee contribution management, banking, maintaining appointment scheduling for clinical and assistant staff and responding to email and telephone inquiries.
- Liaise with other client liaison officers across WMQ's aged care services to establish and maintain clear referral pathways and provide clients with a seamless approach to services delivery.
