Community Development Practitioner

Working closely with the Family Support Manager and multi-disciplinary leadership team of Hummingbird House, the Community Development Practitioner will oversee, guide and advise the implementation of a community development approach to palliative care at the service, so as to identify, develop and more effectively integrate support networks for families receiving care for their child during illness, while dying, and into bereavement. Documentation, analysis and evaluation of this project work, with a view to research publication is also a requirement of the role.

As a senior and experienced practitioner, this role will bring significant expertise to guide the development of community development focused policy, procedures and practice framework across Hummingbird house, and will also lead the implementation of the HH community development work plan across the service.
