Case Manager

Currently seeking experienced Case Manager to work within a dynamic team supporting young people in the South Brisbane region. Full time position 76 hour fortnights fixed term until June 30 2019.
Key Responsibilities Include:
- Working with young people to gain and sustain stable tenancy
- Supporting young people transition from Department of Child Safety Care
- To support young people to build capacity and essential life skills
- Provide Case management within the South Brisbane region
