We are seeking someone to fulfil the role of Intake Worker, who has passion, great organizational skills, great phone manner, excellent written and verbal communication skills, great administration skills and who can work well within a team.  This role will be based at Beenleigh and will be a full time role (76hrs per Fortnight) for a fixed term period of 12months.

Key Responsibilities Include:

  • Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values.
  • Conduct initial eligibility phone assessment with families and individuals from the stated client group to collect information in line with stated objectives. 
  • Provide information, advice and support for families and individuals in the regional sector as well as provide referral information and where appropriate supported referral to services for families and individuals in the regional sector 
  • Provide brief case management intervention and advocacy for families and individuals as assessed to meet their needs.  
  • Attend STARH staff meetings and discuss eligible referrals in line with service capacity.  
  • Maintain and update STARH Project resource information bank.  
  • Produce reports as required and maintain accurate, up to date case notes and statistics.
  • Ensure that STARH activities meet duty of care obligations under the provisions of health and safety legislation and WMQ policy. 
  • Facilitate processes to optimise communication, access to services and linkages between other service providers, community partners and relevant stakeholders to ensure client(s) needs are met. This includes liaising with program development workers at other services who are able to provide needs based programs for the client group. 
  • Assist the Coordinator in the delivery of STARH activities and programs including provide input to the annual budget development process with regard to STARH project objectives.
  • Ensure the needs and outcomes for the target group and STARH objectives are clearly understood by other stakeholders and report on barriers being experienced to service delivery. 
  • Undertake all administrative duties including but not limited to ensuring high quality, up to date case management notes/materials and maintenance of accurate data and update in systems. 
  • Operate within the STARH budget and maintain appropriate receipts and accounts. 
  • Work collaboratively within a team environment, actively participate in regular staff, casework and work review meetings as well as in the training, supervision and support of volunteers and placement students if required. 
  • In relation to other responsibilities accept such other duties as may be from time to time determined by the Coordinator.

    Qualifications, Knowledge and Requirements:
     
  • Tertiary qualifications in human services or a related field.
  • Demonstrated experience in an intake, advice, advocacy and referral role or similarly related position. 
  • Demonstrated experience working with families and individuals at risk of homelessness or homeless within an identified framework or model of practice.  
  • Demonstrated experience using case management principles within a holistic framework.  
  • Knowledge and understanding of tenancy law, particularly the Residential Tenancies and Rooming Accommodation Act; and Residential Tenancies and Rooming Accommodation Regulation.  
  • A good understanding of issues affecting the target group.  
  • Demonstrated high level communication and interpersonal skills including experience in engaging and working with client(s). 
  • Understanding and commitment to social justice principles, confidentiality and privacy.  
  • Demonstrated ability to work with minimal supervision in an environment requiring flexibility and as part of a team. 
  • Experience in establishing and maintaining agency network relationships within the human service environment to better meet client needs. 
  • Demonstrated effective PC skills including MS Office and other related applications packages (including reporting). 
  • Current Suitability Card for Child Related Employment (“Blue Card”) and First Aid Certificate. 
  • Current Driver's Licence and willingness to drive in the course of work. 
  • Relevant probity checks required by legislation and WMQ policy.

Essential Requirements Essential: Must have a current Drivers licence and willingness to drive in the course of work.
