Disability Care Leader - NDIS

About the Role
To effectively and efficiently contribute to the NDIS Home Support Service within the Community Services Directorate of Wesley Mission Queensland. Home Support NDIS is responsible for delivering support to NDIS participants living in the community, with a particular focus on those with high and complex care needs.
As a Care Leader you will focus on leading, supporting and developing Disability Support Workers who are providing support to people with a disability and/or high and complex care needs. This involves ensuring that services are delivered in accordance with Wesley Mission Queensland policies and procedures and align with the NDIS practice standards. The Care Leader is responsible for providing Case Management to people who have NDIS plans. The Care Leader will work with Supported Accommodation Managers to implement synergies across rosters and staff models.
