Case Manager YHARS- Youth Housing & Reintegration Service Full -Time Permanent 


Key Responsibilities Include:

1.     Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values.

2.     Engage families and individuals from the stated client group to provide support in line with stated objectives.

3.     Develop and maintain professional relationships with the client(s) and relevant stakeholders to facilitate assessment of individual needs.

4.     Provide holistic case management, support and advocacy for the client(s) as assessed to meet their needs. This includes developing, planning, implementing and reviewing holistic case management plans with client(s) and providing professional tenancy advice, information, advocacy and support to clients as required and implement in holistic case management planning.

5.     Develop and maintain professional relationships with other services involved with the client(s) and participate in meetings with clients, and or other professionals as required and deemed appropriate.

6.     Facilitate processes to optimise communication, access to services and linkages between other service providers, community partners and relevant stakeholders to ensure client(s) needs are met.  This includes liaising with program development workers at other services who are able to provide needs based programs for the client group.

7.     Produce reports as required and maintain accurate, up to date case notes and statistics.

8.     Ensure that YHARS activities meet duty of care obligations under the provisions of health and safety legislation and WMQ policy.

9.  Ensure the needs and outcomes for the target group and YHARS objectives are clearly understood by other stakeholders and report on barriers being experienced to service delivery.

10.  Undertake all administrative duties including but not limited to ensuring high quality, up to date case management notes/materials and maintenance of accurate data and update in systems.
