Hummingbird House is Queensland's only children's hospice and provides short break stays, family support services, creative therapies, and care at the end of life for children with life limiting conditions and their families. To learn more about Hummingbird House, please ​visit hummingbirdhouse.org.au. Hummingbird House is seeking to recruit a Community Development Worker. 

 

Working closely with the Family Support Manager, Clinical Service Manager, and multi-disciplinary team of Hummingbird House, this position will be responsible for the development and provision of community-focused services to promote efficient, effective, child and family-focused outcomes for children and young people receiving palliative care. The role provides these services to families, community stakeholders and other agencies, both within the hospice and community settings - with a view to building capacity in the broader community, specifically in the areas of death literacy, home based approaches to death care, and non-professionalised bereavement support models.

Essential Requirements Essential: 1. Tertiary qualifications in community development, or equivalent as determined by Hummingbird House team.
Essential: 2. Demonstrated ability and proficiency in the development, delivery and evaluation of community-based initiatives aimed at working with children, young people, and their families.
Essential: 3. Demonstrated ability to provide skilled identification, mapping and connections with local community support agencies and networks.
Essential: 4. Demonstrated experience in developing services, processes and procedures
Essential: 5. Demonstrated experience showing high levels of initiative and ability to work independently to deliver outcomes.
Essential: 6. Knowledge and experience in the application of community development principles in health, or other social support contexts.
Essential: 13. Demonstrated effective PC skills including MS Office and other related applications packages (including reporting).
Essential: 14. Relevant probity checks required by legislation and WMQ policy.
Desirable: 1. Other specialised experience or qualifications in areas of disability services, music, art, complementary therapies, or other skills related to paediatric palliative care will be highly regarded.
Desirable: 2. Specialist experience or qualifications in death care, grief and loss, or bereavement will be highly regarded.
Desirable: 3. Knowledge of public health and palliative care frameworks will be highly regarded.
Desirable: 4. Experience with public speaking and community presentations will be highly regarded.
Essential: 7. Demonstrated high level verbal and written communication skills particularly in establishing and maintaining rapport with staff, children and their families.
Essential: 8. Demonstrated skills in identifying a broad range of needs both within families and across their caring networks.
Essential: 9. Ability to work effectively as a member of a multi-disciplinary team as well as independently, with an ability to be flexible with staff, and collaborate with Multi-disciplinary panel members, and children/families where required.
Essential: 10. Demonstrated understanding of Workplace Health & Safety issues, Equal Opportunity and Anti-discrimination legislation.
Essential: 11. Possession of a ‘C’ class driver’s license and willingness to drive in the course of duties.
Essential: 12. Willingness to work within the Philosophy, Policies and Procedures of Wesley Mission Queensland, and The Uniting Church in Australia.
