Care Manager

This position will be operating within the principles of a Consumer Directed Care model, providing correct information, practical advice, and opportunities for exploring choices, rights and responsibilities of Consumers and their families. This will be underpinned by the Consumer or their advocate self-determining their own goals.
Key Role Accountabilities
- Tertiary qualifications in community, social welfare, health or a related discipline.
- Demonstrated understanding of the principles of Consumer Directed Care.
- Demonstrated experience and ability to coordinate care services with minimal supervision and as part of a multi-disciplinary team including care management and assessment of Consumer's functional, psychosocial and emotional needs within a goal setting framework.
- Highly effective interpersonal, oral and written communication skills.
- Highly effective in organizational and time management skills.
- Sound computer skills with the Microsoft Office suite of applications.
- Understanding and commitment to the principles of Continuous Quality Improvement.
- Demonstrated understanding of the new Aged Care Quality Standards.
- Ability and willingness to work within the philosophy, values and policies of Wesley Mission Queensland and The Uniting Church in Australia.
- Current Driver's Licence and willingness to drive in the course of work.
- Relevant probity checks required by legislation and WMQ policy.
