The Eagleby Community Connect position will work in a high need community to facilitate the support and referrals for individuals and families ('service users') who participate in the Community Connect project to other specialist services – such as those that support people with complex needs and those affected by domestic and family violence. Community Connect aims to promote and encourage resilience in vulnerable individuals, families and communities.

 

The following requirements must be met in the delivery of Community Connect.

  • Facilitate access to services and supports including the capacity for case coordination.
  • Develop close working relationships with local specialist services including the development of networks across the region for referrals, support and assistance.
  • Establishment of appropriate processes to handle referrals that are not accepted to ensure support is provided outside of the program, eg an individual may be referred directly to more appropriate supports in the community.
  • Respond quickly and effectively to arrange emergency assistance if required.
  • Knowledge of legal requirements in relation to issues such as privacy, information sharing, and consent to information about service users being communicated to external supports and services.
     

A vacancy currently exists within the Logan City Youth & Family Services team based at Eagleby Neighborhood Centre. Community Connect worker – full time SCHDSI Award Level 5.

Essential Requirements Essential: Must have a current Drivers licence and willingness to drive in the course of work.
Essential: Tertiary qualifications in Human Services or related field and /or extensive experience working in the Community Sector
Essential: Demonstrated experience in program and service delivery to families and community members.
Essential: Ability to function both as part of a team and autonomously with minimal supervision.
Essential: Demonstrated high level verbal and written communication and interpersonal skills, including report writing, negotiation, advocacy and the ability to effectively engage with clients, families and other service providers.
Essential: Demonstrated understanding of the current issues for communities within Eagleby.
Essential: Demonstrated understanding of Community Development principles and frameworks.
