Care Worker

Wesley Mission Queensland Community Care (WMQCC) aims to encourage and motivate our customers to enjoy life and maintain good health and well-being. The services we offer are designed to facilitate individual choice and maximise independence. Our program provides innovative community and home-based care and support services to a variety of customer's with individual and varying needs. This position will be providing direct support to customer's living in the community.

The role includes but is not limited to personal care (showering/dressing), continence support, assistance with meals, assistance with medications, domestic duties (laundry/housework/ironing/gardening), shopping, transport, in home respite, centre based respite care, overnight respite (including sleepovers), social support/activities, access to other relevant services and any other tasks as stated in the customer care plan.

Key Responsibilities Include:

1. Align one's workplace behaviour in accordance with the WMQ Charter, Values and Mission of the organisation

2. To work as part of a team to provide high quality care services and tasks as stated in the customer's care plan, and in accordance with WMQ's code of conduct and policies and procedures.

3. To consult and/or give feedback to the appropriate Coordinator/Case Manager regarding each customer's changing needs or situations.

4. To recognise and report health and environmental hazards in the customer's home – effecting customers, carers or the extended Team. Further, to assist with corrective actions in consultation with the Coordinator.

5. To provide a customer focused service to customers and to work alongside their carer, families and others as outlined in the care plan and in accordance with WMQ Charter, code of conduct and policies and procedures.

6. To record and submit daily tasks and timesheets accurately (i.e. in live time) via the ComCare system.

7. To drive in a respectful, safe and professional manner in the course of duties ensuring any WMQ vehicles allocated are respected and maintained.

8. To actively participate in performance management reviews, staff meetings, training, support visits and all continuous improvement initiatives.

9. To be flexible and adaptable to changing schedules within your nominated hours.

10. To work in accordance with the values and policies of Wesley Mission Queensland.

Essential Requirements Essential: Must have a current Drivers licence and willingness to drive in the course of work.
