Leisure and Lifestyle Coordinator

The role is a permanent full time position at WesleyCare Jindalee, based at our Sinnamon Village Campus. The Leisure and Lifestyle Coordinator must have basic understanding of NDIS. The successful applicant must be able to assess, plan, implement and evaluate a leisure and lifestyle program for every individual resident. These programs may be group or individually oriented and must provide psychosocial and/or physical stimulation to each resident's interests and level of ability. The programs must also focus on enhancing the lifestyle of each resident by considering their holistic, social, cultural, emotional and spiritual needs.
Key Role Accountabilities: 1. Align one's workplace behavior in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values. 2. To assist residents to attain and maintain their optimum degree of independence and quality of life. 3. Within the scope of qualifications and in collaboration with the multi-disciplinary team, assess, plan, implement and regularly evaluate a range of innovative lifestyle and activity programs to meet the specific needs of the residents and their families, in order to facilitate participation and positive outcomes under the NDIS guidelines. 4. The Leisure and Lifestyle Coordinator will be responsible to consult with all residents individually in developing and implementing activities of their choice within approved NDIS plan and maintain effective relationships with residents, families, staff, support coordinator and external providers. 5. Prepare and lead individual and group social and recreational activities encompassing the cultural, emotional and spiritual needs of the residents and where appropriate implement programs with a community involvement focus. 6. Assist in the education, co-ordination and supervision of volunteers. 7. Identify and respond to individual needs of residents as identified in approved NDIS plan and current care plan. 8. Liaise with appropriate staff members, support coordinators, external providers and relatives regarding resident care, progress and community participation. 9. As required, professionally document and report in Leecare - NDIS Activity groups and individual community participation and ongoing progress of residents and programs. 10. Ensure the team, activities and programs are in accordance with relevant WMQ policy and procedure. 11. Attend and participate in regular meetings with residents and other Leisure and Lifestyle Coordinators to enhance program delivery. 12. In consultation with the local Manager for rostering purposes for NDIS community participation. Maintain and order necessary equipment in line with operational and budgetary constraints. Qualifications, Knowledge and Requirements: Essential: 1. Qualifications of Cert IV minimum and relevant experience in Leisure and Lifestyle 2. Demonstrated experience working with people with a disability. 3. Demonstrated experience in the planning, implementation, facilitation and ongoing review of innovative and flexible recreational programs. 4. High level of verbal and written communication skills and experience developing professional rapport with clients. 5. Current First Aid Certificate. 6. Knowledge of and competency using Microsoft Word and other web based platforms. 7. Demonstrated ability to work professionally with a small multidisciplinary team. 8. Other specialised qualifications in areas of disability, music, art, aromatherapy or other skills related to the work of Leisure and Lifestyle will be highly regarded. 9. Ability to acquire or a demonstrated knowledge of work health and safety regulations and continuous quality improvement practices. 10. Current Driver's Licence and willingness to drive in the course of work. 11. Relevant probity checks required by legislation and WMQ policy.
