CASE MANAGER

YOUTH SUPPORT SERVICES LOGAN
FULL TIME
38 HOURS PER WEEK
LEVEL 5
Key Responsibilities Include:
1. Align ones workplace behaviour in accordance with the principles, values and Mission of the organisation. 2. Work from a strengths-based perspective to assist young people and families with complex issues to meet their identified needs. 3. Assess and case-manage young people creating appropriate case plans and interventions as needed. 4. Assist young people to identify, develop and improve living skills, and assist them to link with local support networks. 5. Support and facilitate access to appropriate support groups and other referral sources in relation to their identified needs. 6. Develop and maintain partnerships with local internal and external stakeholders. 7. Maintain comprehensive and up-to-date client documentation as required under the terms of the funding contract, and within organisational policy. 8. Provide regular reporting to the Management on relevant areas of program functioning and monitor performance indicators. 9. Participate in program development, evaluation and other quality related initiatives including accreditation activities and data collection. 10. Follow risk management policy and processes identified for the program.

Essential Requirements |
Essential: Tertiary Qualifications in Human Services and/or related disciplines Essential: Demonstrated experience working with “at risk” young people who are experiencing complex needs Essential: Demonstrated experience using case management principles within strengths based holistic framework Essential: Demonstrated high level verbal, written communication and interpersonal skills, including case notes, report writing, negotiation, advocacy, and the ability to effectively engage with clients, families and other service providers Essential: Knowledge of the Community Services sector including a willingness to engage with services to provide collaborative working dynamics and positive outcomes Essential: Ability to function as a member of a multi-disciplinary team with minimal supervision Essential: An understanding of the importance of personal self-care in the challenging work environment demonstrating healthy work boundaries Essential: Current Suitability Card for Child Related Employment (“Blue Card”) and First Aid Certificate Essential: Current Driver’s Licence and willingness to drive in the course of work Essential: Relevant probity checks required by legislation and WMQ policy |
