About the Role
This is a Permanent Full-Time position of a Floating Care Leader to effectively and efficiently contribute to the delivery of Home Care Services within the Community Care and Inclusion Directorate of Wesley Mission Queensland (WMQ). Travel across Brisbane and greater areas is a requirement of the role.
This position will operate within the principles of Consumer Directed Care. You will provide effective support, coordination, information, advice and case coordination of customers with Home Care Packages. Provide leadership, support and development of Care Workers who are delivering services to people with aged care home support funding in accordance with WMQ policies and procedures and align with the Aged Care Quality Standards.
Key Responsibilities Include
· Providing leadership and support to Care Workers (Aged Care services).
· Ability to coordinate care services with minimal supervision including case management and assessment of Consumer's needs within a goal setting framework.
· Provide excellent case management to case load of home care package clients while existing Care Leader is on leave.
· Respond to client enquiries and client requests promptly and comprehensively
· Document and record comprehensively so handover is seamless and detailed when Care Leader returns to work.
Skills and Experience
· Certificate IV in Case Management/Service Coordination or other relevant qualifications/experience. (We will consider applicants working towards these qualifications)
· Demonstrated understanding of the principles of Consumer Directed Care.
· Highly effective interpersonal, oral and written communication skills including the ability to develop and maintain effective relationships with a wide range of stakeholders.
· Highly effective organisational and time management skills.
· Sound computer skills with the Microsoft Office suite of applications and client databases.
· Current Driver's Licence and willingness to drive during work.
· Ability to provide excellent, flexible and holistic care to a variety of customers with varying needs.
· Understand and commitment to the principles and practices of privacy, confidentiality and professional boundaries in relation to all customers and staff.
· Comprehensively insured Motor Vehicle, Current Driver's Licence and willingness to drive and use own vehicle (if required) in the course of work
