Engagement Officer

The Home Support NDIS Service sits within the Community Care and Inclusion Directorate of Wesley Mission Queensland. This is a new service being established in the Directorate with the initial concentration on transferring clients, who are eligible for services under NDIS, from other community based support services into the Home Support NDIS service. Home Support NDIS will also attract and transition participants from external referral sources. Home Support NDIS will be responsible for delivering support to NDIS participants living outside of residential care, with a particular focus on those with high and complex care needs.
Key Responsibilities Include:
- Align one’s workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland’s Vision, Mission and Values.
- Promote Wesley Mission Queensland’s services and facilities to prospective clients, community agencies and the wider community.
- Determine prospective client’s eligibility to receive supports from the Home Support NDIS service and prioritise referrals.
- Conduct assessments to verify the individuals support needs and to confirm the services being requested. This includes assessments for those with high and complex care needs.
- Sensitively gather information from individuals, their families and networks to ensure that all disability and medical requirements are considered and risks identified.
- Plan and establish holistic supports in collaboration with the individual and their networks to ensure supports are appropriate and assist the individual to pursue the goals identified in their NDIA plan.
- Prepare quotes and service agreements.
- Assist people with a disability to access the NDIS (navigation of the application process, completing of documentation, gathering diagnostic and functional evidence and identification of goals).
- Maintain accurate electronic records that comply with data collection standards.
- Undertake other duties consistent with the position where required and/or requested by management.
