Clinical Nurse Consultant / Clinical Coordinator

John Wesley Gardens is a state of the art community that offers premiere aged care living at its finest. Our residents are our priority and are provided with excellent Customer Service and the best of care from our friendly, professional staff. The home has 138 beds, a Chapel, Wifi access, spacious gardens and Cafe facilites.
We are seeking an experiened nurse with proven leadership skills and who posesses a passion to provide excellent customer service. The successful candidate will pride themselves on being a team player, be a dedicated professional and will embrace our Philosophy and our Values.
- Current AHPRA Registration with no restrictions
- Must have 3-4 years Registered Nurse experince (Residential Aged Care Experience preferred)
- Up to date immunisations
Key Responsibilities Include:
- Align one’s workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland’s Vision, Mission and Values.
- Contribute to the development, implementation, monitoring and review of resident care to meet the needs of residents and to promote quality of life.
- Provide compassionate, ethical, quality and comprehensive direct care to residents including individual case management and care plan reviews and ensure these reflect the holistic needs of the individual.
- Provide supervision for medications administration in rostered home or as required in other nearby designated homes. It is the responsibility of the designated registered nurse to take legal responsibility for medication management.
- Significantly contribute to case conferencing regarding care planning, implementation and evaluation. Liaise with and role model professional communication with family and other stakeholders.
- Supervise and monitor care practices to ensure competent, contemporary and safe work practices are upheld by all.
- Delegate, coordinate, oversee and collate all relevant ACFI documentation prior to final submission.
- Prepare, supervise and review all resident care data including assessments and care directives and care planning.
- Engage in ongoing professional education and research in generalist practice and specialist areas and integrate learning into work practices of self and others. Specialist areas including but not limited to aged care, young people with significant disability, palliative care, pain, wound management, infection control.
- Ensure all rights, dignity and confidentiality of residents are maintained in all matters.
- Assist in the orientation and ongoing development of staff as well as participate in performance development for self and others as appropriate.
- Achieve outcome standards and service delivery practice is in accordance with Government regulations, funding bodies and other relevant stakeholder requirements including resident privacy and confidentiality.
- Contribute to continuous quality improvement and accreditation processes to ensure a high standard of practice and service delivery within the philosophy and framework of Wesley Mission Queensland and The Uniting Church of Australia.
