Community Access Facilitator

Position Summary:
In line with the implementation of the NDIS oversee, develop and implement community access experiences that are the choice of residents, either internally or externally. These activities must be aligned with the individual community access goals and be within each resident's community access budget. These activities must provide choice, flexibility and spontaneity.
This Community Access Facilitator position will be at a brand new facility located at Mt Gravatt which will be opening in January 2021 and also later at Cannon Hill in February 2021. There is a Permanent Part Time position available to be shared across the two sites once they are open, hours are currently unspecified. This accommodation is part of Support Accommodation, which provides facilitated care to young people with a disability who have high care needs in a physical and social environment that is suitable for their age group. Each accommodation site has the capacity to house 4 residents. Each resident will have their own 2 bedroom apartment which includes a kitchen, laundry, lounge and dining within each apartment. There is also a shared space area within the accommodation.
Key Responsibilities Include:
1. Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values. 2. Oversee the development of NDIS Community Access programs and plans for each resident. 3. Liaise with residents/primary support networks to develop and implement and review NDIS Community Access plans monthly. 4. Develop, submit and oversee NDIS Community Access Service Agreements. 5. Document and report ongoing progress and budget of resident's Community Access programs. 6. Complete risk assessments for all new community access activities for individual/groups of residents and review existing risk assessments annually. 7. Engage and train personal carers / volunteers to provide a high quality community access experience either in house or in the community. 8. Access and support volunteers and link them to the needs and interests of residents. 9. Oversee and mentor personal carers /volunteers involved in the community access program. 10. Liaise and engage with Support Coordinators in the development of the Community Access Program for each resident. 11. Liaise and engage external organisations/providers to meet the individual needs of residents, so they can attend mainstream community access activities, with appropriate supports either internally or externally. 12. Ensure the team, activities and programs are in accordance with relevant WMQ policies and procedures. 13. Attend and participate in regular meetings held by the Community Access Coordinator and other Community Access Facilitators to enhance program delivery. 14. In consultation with the local Manager order and maintain necessary equipment in line with operational and budgetary constraints.
