INTAKE WORKER

The Intake Worker works within the YHARS team, supporting young people at risk of homelessness within the South-Western suburbs of Brisbane. The YHARS program works with young people aged 12-21 years who may have multiple-complex needs, transitioning from Child Safety Orders or youth Detention Centre, and are at risk of homelessness. This role is responsible for overseeing and conducting the intake, assessment and referral services for eligible and non-eligible clients referred to the program.
The role is a Fixed Term Full-time role, based at Ellen Grove.
Key Responsibilities Include:
1. Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values.
2. Assist the Coordinator in the delivery of YHARS activities and programs and provide general administration support.
3. Conduct initial eligibility phone assessment with potential consumers, determine eligibility and provide initial information, referral advice, and other appropriate brief intervention support.
4. Liaise, consult, meet and collaborate with stakeholders to increase access to appropriate service responses for YHARS consumers.
5. Provide brief intervention support via case management to consumers to enable the service to respond to the emerging needs of consumers.
6. Provide intervention and advocacy for clients as assessed to meet their needs to aim to sustain their tenancies.
7. Maintain and update YHARS resource database of information to ensure that all referral sources are up to date and relevant.
8. Produce reports as required and maintain accurate, up to date professional case notes and statistics.
9. Monitor compliance with policies and funding guidelines, and provide advice and regular reports to the YHARS Coordinator.
10. Active participation in training and supervision of volunteers and placement students, if required.
